Cincinnati State Student Vaccination Incentive Program – Spring Semester 2022

Cincinnati State is striving to provide a safe campus experience for everyone.

Spring Semester 2022 students can enter the Vaccination Incentive Program to be eligible for weekly prizes.

Students attending Spring classes are eligible if:

• You got your first or second COVID-19 vaccine shot after Dec. 14, 2021
• You got a COVID-19 booster shot any time

The Spring Student Vaccination Incentive Program ends April 27, 2022.

A free on-campus vaccination clinic (facilitated by Walgreen’s) will take place Wednesday, Mar. 23, from 9 a.m. to noon in ATLC 201.

• Walk-in visitors (students and College employees) can get a COVID-19 1st dose, 2nd dose, or booster shot (if eligible) of the Pfizer vaccine.
• If you’re seeking a 2nd dose or a booster shot, bring your vaccination card.

Already vaccinated and/or boosted?

If you received your first or second COVID vaccine after Dec. 14, 2021, or you obtained your COVID booster shot any time, you can receive a $25 gift card just by entering our Vaccination Prize Drawing— and when you enter the drawing, you could win prizes including:

• 10 weekly prizes of $100, given each week from Mar. 2 to Apr. 27, 2022
• 2 weekly prizes of $250, given each week from Mar. 2 to Apr. 27, 2022
• 1 grand prize of $5,000, given on Apr. 27, 2022

To enter the Prize Drawing:

Fill out the Student Vaccination Incentive eForm and attach a scanned copy of your vaccination card showing that you received your first, second, or booster shot (as applicable).

When you submit the eForm you’re entered in the Prize Drawing, even if you don’t pick up a gift card.

To get your Gift Card:

Clifton Campus: Gift Card pick-up hours and locations are:

• Monday through Friday – 9 a.m. to 5 p.m. – Go to the Student Activities Office (ATLC 204)
• Tuesday and Thursday – 5 to 6:30 p.m. – Go to the Executive Offices (ATLC 343)

Show a staff member your student ID (Surge Card). After a quick check to make sure you’re a registered student, you’ll get your $25 gift card.

Middletown Campus: Go to the Middletown Campus Front Desk, Monday through Friday, 8 a.m. to 4 p.m.

Not yet vaccinated?

Get your first or second COVID-19 vaccine shot, or get your COVID-19 booster shot, from any authorized provider. Then complete the eForm (including a scanned copy of your vaccination card) and you’re eligible for a $25 gift card and entered in the prize drawing.

A Vaccination Clinic on Clifton Campus will take place Mar. 23 from 9 a.m. to noon in ATLC 201.

Prize Drawing Dates

The Vaccination Incentive Program begins Feb. 22, 2022, and ends Apr. 27, 2022. The last day to enter (by filling out the Prize Drawing eForm) is Apr. 26, 2022.

Prize Drawing dates and prizes to be awarded are:

• Mar. 2, 2022 – 10 prizes of $100 & 2 prizes of $250
• Mar. 9, 2022 – 10 prizes of $100 & 2 prizes of $250
• Mar. 16, 2022 – 10 prizes of $100 & 2 prizes of $250
• Mar. 23, 2022 – 10 prizes of $100 & 2 prizes of $250
• Mar. 30, 2022 – 10 prizes of $100 & 2 prizes of $250
• Apr. 6, 2022 – 10 prizes of $100 & 2 prizes of $250
• Apr. 13, 2022 – 10 prizes of $100 & 2 prizes of $250
• Apr. 20, 2022 – 10 prizes of $100 & 2 prizes of $250
• Apr. 27, 2022 – 10 prizes of $100 & 2 prizes of $250, & 1 grand prize of $5,000

A student who wins a prize of $100 or $250 remains eligible for the $5,000 grand prize, but is not eligible for additional weekly prize drawings.

Weekly winners will be notified by the Provost’s Office. Prize winners will need to come to the Provost’s Office (ATLC 343) with your student ID to pick up your prize.

Cincinnati State Vaccination Incentive Program Official Rules

By entering the Vaccination Incentive Program (“Promotion”) offered by Cincinnati State Technical and Community College (“Cincinnati State” or “College”), you agree to be bound by these official rules.

Eligibility

This Promotion is open to Cincinnati State students registered for Spring Semester 2022 who receive a COVID-19 first or second dose after December 14, 2021, or students who receive a COVID-19 booster dose any time. Students may enter the Promotion at any time until April 26, 2022.

Individuals who cannot be vaccinated due to medical conditions may also enter the Promotion. For more information on obtaining a medical exception go to the Cincinnati State Office of Disability Services.

Definitions

Vaccinated

To be considered vaccinated, a student must receive one or two doses of the Pfizer or Moderna vaccines after December 14, 2021; one dose of the Johnson and Johnson vaccine after December 14, 2021; or receive a World Health Organization approved vaccine. To be considered vaccinated and boosted, a student must receive a booster dose of any approved vaccine.

Students

Cincinnati State students registered for Spring 2022 classes, including concurrently enrolled high school students. College employees who are also registered students are eligible to participate in the Prize Drawing.

Promotion Dates

The Promotion begins February 22, 2022, and ends after the last contest drawing on April 27, 2022. Entries received after 5 p.m. EST on April 26, 2022, will not be considered for awards, regardless of whether the late receipt resulted from human, computer or transmission error. Vaccine doses or booster doses must be administered by the above 5 p.m. EST deadline.

Entry Process

The Promotion is open to vaccinated Cincinnati State students who are registered for and attending classes in Spring Semester 2022.

To participate in the Promotion, students must complete the designated Promotion eForm. Students may enter as soon as they meet the eligibility requirement of being vaccinated (first or second dose, received after December 14, 2021) or received the booster dose. Vaccination status and Cincinnati State registration status must be verified to receive a prize.

Individuals who have not been vaccinated can receive the vaccination or booster, as applicable, on the Cincinnati State Clifton campus on designated days/times or may receive the vaccination from any authorized COVID-19 vaccination provider.

Prize Drawings

Winning names will be randomly selected by Cincinnati State staff each Wednesday from March 2, 2022, through April 27, 2022.

No student may win more than one prize, except for the grand prize of $5,000, which may be won by a previous winner of a $100 or $250 prize.

Students awarded the grand prize in the Fall 2021 Vaccination Incentive are not eligible for additional prizes in Spring Semester.

Notification of Winners and verification of prize eligibility

Prize winners will be contacted by the Cincinnati State Provost’s Office after their status as a registered student has been verified.

The student prize winner must bring their student ID card to the Provost’s Office (ATLC 343, Clifton Campus) for verification in order to receive their prize. The name and prize may be announced publicly on the Cincinnati State website and social media channels, and/or in other College communications. If a winner does not respond to notification of the win within five (5) business days, the winner will be disqualified and a new winner will be chosen.

Conditions of Participation

Use of Submissions

By entering this Promotion, the student hereby grants to Cincinnati State the right to take their picture and to reproduce, use, exhibit, display, broadcast, distribute, modify, adapt, and create derivative works of photographs, videotaped images or video/audio recordings of the Contestant by incorporating them into publications, catalogues, brochures, books, magazines, photo exhibits, motion picture films, videos, electronic media, web sites, and/or other media, or commercial, informational, educational, advertising, or promotional materials or publications related thereto.  

Cincinnati State reserves the right to modify or change the Vaccination Incentive Program rules at any time. The Promotion is subject to all applicable United States federal, state, and local laws and regulations. 

Contest Ownership

Cincinnati State is the sole owner and operator of the Vaccination Incentive Program. Official rules, terms and eligibility requirements, prize information, and selection are administered by the College.  

Waiver of Liability 

By hosting this Promotion, Cincinnati State in no way endorses or sponsors any manufacturer of vaccinations. By entering this Promotion, the student is choosing to voluntarily receive the vaccination(s) and assumes all liability therefore. Cincinnati State is not responsible for any injuries or illnesses occurring as a result of a student obtaining vaccinations and entering this Promotion.

Additionally, any prize may be taxable and winners are responsible for payment of any taxes.

Students may not receive financial aid and other resources in excess of their annual cost of attendance at Cincinnati State. The Cincinnati State Office of Financial Aid will review the student winners, determine students who exceed their cost of attendance, and work directly with these students to maximize their financial aid.
